thanks for your feedback. Indeed I think that calibration of WV3 images is
not yet supported by OTB. I've opened an issue as a feature request on OTB
gitlab:
https://gitlab.orfeo-toolbox.org/orfeotoolbox/otb/issues/1665
Don't hesitate to comment or complete this issue. I suspect that OTB
silently fallback to the WV2 metadata values tabulates in the code which
generates this weird output that you've got.
Regarding your second question, the gain and the offset are indeed read and
use by OTB internally (in the otbImageToradianceImageFilter class) to
convert digital number to radiance. It is the way it is done for all
sensors. For WV3 more specifically it should be check like for the relative
spectral responses if those values are correctly retrieved from the IMD
metadata file.
I've tried to describe all these points in the issue.

I forgot to mention also that in the Technical description, the following
formula is used for converting into Radiance.
It has the absolute calibration factor and the effective bandwidth
included. These parameters are provided for each band in the .IMD file, but
I didn't see them in the formulas used by OTB calibration.
